Plans of Safe Care
Plan of Safe Care (POSC) is a free voluntary program offered to help families improve the health of their children from pregnancy and forward. The Plan of Safe Care team will work with you to focus on the health and development of your baby.
You may be referred to Plan of Safe Care if you or your baby tested positive for a controlled substance during pregnancy or at birth, if your baby is experiencing signs of withdrawal after birth or if your baby is displaying signs of Fetal Alcohol Spectrum Disorder (FASD).
The Plan of Safe Care program is available to any caregiver of an infant who has been affected by substance use and is up to one year of age.
How We Help You and Your Family
We are on your team to help you with any needs you identify at enrollment or any time in your baby’s first year. Our incentives are based on the needs of the baby and are used to support the baby developmentally in the first year of life.
Program Details
Location: Bi-Weekly In-Home Visit

✓ Increase the likelihood of obtaining and maintaining sobriety
✓ Increase clients' positive relationships and bonding with their children through their recovery journey
✓ Increase clients' skills to improve communication and identify stressors
✓ Increase access to family and community support
✓ Successfully reunify families whose children have been removed from the home and prevent any future child welfare incidents

What to Expect
Plan of Safe Care Coordinator
You will be assigned a POSC coordinator who will support you and your family and
help with maintaining successful recoveryResource Connection Identifying
The needs of you and your baby and connecting you to community resources that will help meet those needs
Multidisciplinary Team Meetings
Your team will meet monthly to update the goals in your Plan of Safe Care and communicate any new goals/needs
Incentives
The Plan of Safe Care program includes incentives to assist your baby with hitting milestones throughout the first year of development
Translation Services Available
If you need help with translating any programs or content within Erie Family Center, please complete the form and a staff member will be in touch. Translations available include, but are not limited to: Spanish, Bosnian, Russian, Ukrainian, Arabic, Vietnamese, Nepali, Swahili, Somali, Burmese, Pashto & Dari.
All of our programs are funded through the PA Office of Child Development and Early Learning, Erie County of Children and Youth, and free to all residents of Erie County.
